A twelve-year-old Cavalier King Charles Spaniel named Biscuit came into my consult room because he’d started bringing his dinner back up within a minute of finishing it — undigested, tube-shaped, coated in mucus, no retching beforehand. His owner had already tried three foods. What Biscuit had wasn’t a food problem at all; his esophagus had lost tone, a change we confirmed on imaging, and no amount of diet-swapping was ever going to touch it. That case is the reason I open every conversation about post-meal vomiting with the same question: is this actually vomiting, or is it regurgitation wearing vomiting’s clothes?

The fork owners miss: regurgitation or true vomiting

These look almost identical from across the kitchen, and that’s exactly the problem. According to the American Kennel Club, regurgitation is “a passive expulsion of food or fluid without any nausea, salivation or retching,” and dogs “usually regurgitate right after eating,” often bringing up food that’s still tube-shaped and undigested. Vomiting, by contrast, is an active, muscular event — abdominal contractions, drooling, lip-licking, sometimes retching for a minute before anything comes up — and it can happen soon after a meal or hours later.

I don’t treat that distinction as trivia. It’s the fork in the road that determines everything downstream: whether we’re looking at the esophagus or the stomach, whether the fix is a feeding-posture change or a full workup. Regurgitation that shows up consistently, especially in a young dog or an older one with new muscle weakness, deserves imaging — a barium study or fluoroscopy to look for megaesophagus or a vascular ring anomaly. True vomiting, especially with retching and nausea, points somewhere else: dietary indiscretion, a toxin, pancreatitis, or exocrine pancreatic insufficiency (EPI).

When the stomach, not the esophagus, is the problem

If your dog is genuinely vomiting — retching, drooling, food coming up partially digested and acidic-smelling — the differential shifts toward the stomach and pancreas. One condition I see under-recognized is EPI, where the pancreas stops producing enough digestive enzymes. A recent review in the Journal of the American Veterinary Medical Association notes that “EPI is treated by pancreatic enzyme replacement therapy, nutritional management (low-residue diets with moderate fat content), and supplementation of cobalamin” (JAVMA). That same review is candid about the limits of current knowledge, adding that “future studies are needed to further understand the causes of persistent dysbiosis in animals with EPI following initiation of pancreatic enzyme replacement therapy and assess the efficacy of treatments to ameliorate these abnormalities” — which is a fair summary of where this field actually stands: enzyme replacement helps, but the gut microbiome doesn’t always snap back cleanly.

Toxic ingestions are the other stomach-driven cause I never want an owner to miss, because the treatment window is narrow. The gut microbiome angle, and where the evidence actually stops

Owners often ask me whether a probiotic will settle a dog’s stomach after a bout of vomiting. My honest answer, mechanism first: a probiotic may help support the gut’s return to normal function once the acute cause is addressed, but it is not a substitute for finding that cause. A 2025 trial in Frontiers in Veterinary Science looked at dogs receiving probiotic support and found that “diversity metrics did not distinguish non-responders from responders” (Frontiers in Veterinary Science) — in plain terms, the researchers couldn’t predict from microbiome diversity alone which dogs would improve. That’s not a reason to dismiss probiotics; it’s a reason to hold the claim to what the data actually shows instead of promising a clean fix. Here’s a short one, because it matters on its own: speed of eating is probably the single most under-addressed cause of post-meal regurgitation in an otherwise healthy dog.

In my practice, the dogs I see most often for “he throws it right back up” are the fast eaters — Labradors, especially, and any dog fed in a multi-dog household where there’s competition at the bowl. I remember a four-year-old Lab named Otis, 78 pounds, whose owner described him finishing a two-cup meal in under thirty seconds, then bringing most of it back up on the kitchen tile within a couple of minutes, still recognizably kibble-shaped. We didn’t image Otis first — his history alone told the story, and it wasn’t neurological. According to Preventive Vet, slowing consumption speed with a slow-feeder bowl or by spacing meals into smaller portions is a first-line, low-risk intervention for exactly this pattern, and it’s usually where I start before reaching for anything else. Otis went from daily regurgitation to occasional, over about three weeks, once his owner switched to a slow-feeder and split his meals into three smaller sittings instead of one.

Where I land, and the counterargument I take seriously

My position is that every dog with new, persistent post-meal vomiting or regurgitation deserves both ends of the diagnostic spectrum considered — the imaging study for the ones where the cause is structural or neurological, and the slower, more integrative tools like feeding management and gut support for the ones where it’s mechanical or dietary. The counterargument I hear most is that imaging is expensive and unnecessary for a dog that “probably just ate too fast.” I take that seriously — most dogs are exactly that, and I don’t image every fast eater. But I do image the ones where feeding changes fail after two to three weeks, where regurgitation appears in a young dog with no obvious behavioral cause, or where there’s any hint of weight loss, cough, or aspiration risk. The cost of missing megaesophagus or a vascular ring anomaly is far higher than the cost of a barium study.

The practical rule I give owners

Watch the pattern for a week before you decide it’s benign. If food comes back up within minutes, undigested and without retching, try slowing the meal down first — a slow-feeder bowl, smaller portions, more frequent sittings — and note whether it improves within two to three weeks. If instead there’s retching, drooling, or the vomit looks digested or bile-tinged, or if the dog is losing weight despite eating well, that’s a stomach-and-pancreas conversation, not a bowl conversation, and it belongs in front of your veterinarian. Either way, resist the urge to change food three times before you’ve even sorted out which of these two processes you’re actually watching.
